TUITION/ PAYMENT

Students are on a month-to-month contract. Tuition for the month is due in full the first week of every month by check, credit card, or cash. Payment must be made for the entire month in advance to reserve your guaranteed lesson time for the upcoming weeks. There is a $5 late fee for each week that tuition is not received. If payment is not received by the third week of the month, lessons will be temporarily suspended until all charges and fees are resolved.

CANCELLATION POLICY

Cancellations must be given at least 24 hours in advance to receive a make-up lesson that is to be taken within the month of the cancellation. If the parent/student fails to comply with the 24-hour cancellation policy, the lesson will be considered a loss for the student and the studio is not obligated to refund or reschedule the lesson.

 

In the event of an allowed make-up, parents will be given reasonable notice of a possible make-up time; however, if the student’s schedule does not permit for the make-up lesson, the lesson is a forfeit. Credits will be made for emergencies and extenuating circumstances only and at the discretion of the studio. If the instructor is unable to attend a lesson you will be given sufficient notice and either a credit or make-up lesson.


If for some reason, you the parent must conclude lessons entirely, we require a two-week notice in order to end our lessons in a positive manner. We will not be able to provide credits or refunds for the remaining weeks of our monthly contract.

 

TARDINESS

Students are expected to arrive on time. The studio will allow a window of 15 minutes. After the allowed time, the lesson is lost and no make-up will be given. We cannot offer a full class time if students arrive late due to our studio schedule. Exceptions will be made for weather and extenuating circumstances only. 

SCHEDULE REQUEST

Please be courteous of the instructors’ schedules and of other families in requesting frequent changes to the schedule. We will do our best to accommodate everyone, however, most lessons occur during a very limited amount of hours after school and are at times very difficult to reschedule.

RECITAL PARTICIPATION

Student participation in piano recitals is encouraged and will assist the student in setting future goals. Two major recitals are given each year during the winter and spring months. Students will be invited to perform during each recital contingent upon consistent practice. Students are also required to perform a piece of their choice at the beginning of each class. At the end of the class, students will perform another short piece learned during each week’s class time.

OTHER MUSICAL PARTICIPATION

Competitions, festivals and achievement days are available to all students in our studio. Suggested participation in these events may depend on the student’s level of progress, involvement and dedication to practice.

LESSON SUPPLIES

We provide materials including sheet music, theory worksheet, and lab access to our students at no extra charge. However, students are required to bring a binder to collect all lesson materials, including sheet music, theory worksheets, and teacher comment chart for weekly review. Students perform best when reviewing weekly instructions and individual comments so please make sure to bring your binder each week to class.

PARENT EXPECTATIONS

Parents/guardians of young children are welcome and encouraged to join and participate in each lesson when at all possible. The involvement of each parent is necessary to ensure that the student may enjoy the piano within a healthy musical atmosphere. Regular practice outside of the classroom is essential for musical progress. Supervised and constructive practice time spent between the parent and student daily is necessary for the progress of the student. Parents of young children or beginning students are required to set aside 15-20 minutes per day at least five days a week. A signature will be required in the box (attached to each lesson packet) for each day the student has practiced. Failure to practice regularly will result in a warning. If a student goes two weeks without regular practice, the student will be dismissed from classes without a refund.

 

Parents/guardians of older children are asked to assist their child by assigning a regular practice schedule at home. They will be asked to check their assignment notebook regularly to ensure that all aspects of their studies have been practiced. Parents may be asked to discuss and listen to their child’s weekly assignments the day before their scheduled lesson. Upon completion of the child’s weekly demonstration for their parent, the parent will be asked to sign the assignment book. Consistent practice time is a must in seeking progress.

Waiver of Liability

In consideration of being allowed to participate in Lee’s Academy of Music Classes (the “Activity”) offered by Lee’s Academy of Music. (“LAM"), I represent, covenant, and agree, on behalf of myself and my heirs, assigns, and any other person claiming by, under or through me, as follows:

  1. I acknowledge that participation in the “Activity” involves certain risks and that injuries, death, property damage or other harm could occur to others or me/my child. I accept and voluntarily incur all risks of any injuries, damages or harm which arise during or result from my or said minor child participation in the “Activity”, regardless of whether or not caused in whole or in part by negligence or other fault of “Company”, it affiliates, employees, officers, agents or insurers (“Released Parties”).

  2. I do forever waive, release, absolve, indemnify and agree to hold harmless LAM, its owners, officers, employees, agents and all others acting on its behalf from any and all claims of liabilities for injuries or damages to my or participating minor child’s person and/or property, including those caused by negligent acts or omission of any of those mentioned or others acting on their behalf arising out of or connected with my or minor child’s participation in this activity.

  3. I agree to allow the LAM and its affiliates, the use of my and/or said minor child’s name and likeness in connection with this activity, for any lawful related purpose in perpetuity in all forms of media, including photo, video or film, now and forever known. I understand that at such events or related activities I and/or said minor children may be photographed or video graphed. I assign LAM, the rights and permission to use those for any social media and promotion including but not limited to photographs, videos, audio, promos that include me and/or said minor child during such activities. Any consideration for exception must be made in writing.

  4. I agree that deposits, class tuition, and fees remain non-refundable if I choose to withdraw my participation.

  5. By registering myself or my minor child, I acknowledge and represent that I have read and reviewed this Waiver, Release and Hold Harmless Agreement, fully understand, and accept its terms, and accept it voluntarily. In case of emergency, I agree to allow the above parties to call for emergency medical assistance and I am aware that I am financially responsible to those medical services.

Studio Policy

Rental Policy

Piano Rental is available to enrolled students of Lee’s Academy of Music. A standard digital piano equivalent to a YAMAHA P-45, valued at $499, will be delivered to your place. The digital piano will be supplied for use when first month of tuition is paid through the online payment page. Lee’s Academy of Music confirms that the piano is in excellent condition and is free of known distress. The renter is aware this piano is used and that any damage caused by you or because of the piano is not the liability of Lee's Academy of Music. Any damage to the piano during the rental period is the sole responsibility of the renter. The renter will be required to check the piano in and out with representative of Lee's Academy of Music at the beginning and end of the rental period. The piano rental begins on the date of the students' enrollment and ends when the piano is returned on or before the student chooses to terminate lessons. Any controversies or claims arising under, or relating to this agreement shall be governed by the substantive law of the state of Utah. Lee’s Academy of Music reserves the right to terminate the rental agreement under conditions including late tuition payment or student tardiness.

 

Acceptance of proposal: The above policy, specifications, and conditions are satisfactory and are hereby accepted. This policy may be changed or altered at any time by Lee’s Academy of Music without notification. By signing the registration form, you confirm that you have seen the piano and have noted any pre-existing imperfections with representatives from Lee's Academy of Music.

Checklist:

  1. A monthly non-refundable $5 insurance fee will be applied to your tuition payment.

  2. Other than normal wear and tear, you may be charged repair and replacement fee if damaged.

  3. No objects are to be rested on the piano lid including but not limited to drinks, center pieces, and instruments.

  4. Your instrument can be returned at any time. However, there is no refund for tuition because tuition is not pro-rated. 

  5. Please provide at least 48 hours in advanced if you need to return the piano. (Termination of classes are still subjected to the 2-week advanced notice.)
